Agent Contracting Instructions and Checklist

You are contracted, not appointing, with the company to market, solicit, and sell MedPoint Advantage.

$5.00 Contracting Fee is required for each Agent
(This is deducted from your first commission statement)

Step #1

Complete the entire Agent Contract Application and submit to the Company.

If you are a sub-agent, please complete and submit the Sub-Agent Contract Application instead.

You Cannot Enroll Anyone in MedPoint Advantage
Until You have Completed Step #2.

 

Step #2

Upon approval, you will be notified by e-mail your Agent Writing Number. You will need your Agent Writing # in order to access the Agent Contract Page to complete your Agent Contract. Complete the Agent Contract and Agent Agreement of Understanding and submit to the Company.

Until You have Completed Step #2.
